Who is responsible
CK General Practice, Unit 8/9 Boxer Junction, 26 Archbishop Desmond Tutu Rd, Klerksdorp 2571. We are the responsible party for the information described here, as that term is used in the Protection of Personal Information Act. Our Information Officer is Dr S. What we collect, and why
The enquiry form asks for your name, your phone number, your email address if you choose to give one, when you would like to come in, and the general reason for your visit. We use it to reply to you and arrange your appointment.
The form does not ask for medical details, and you should not put them there. Bring those to your appointment, where they belong in your record.
Who else sees it
Your enquiry reaches the practice through our Google Workspace account and is read by the practice alone. Google processes it on our behalf under terms that hold them to the standard of protection this Act requires, including where their systems sit outside South Africa.
We do not sell your information, and we do not share it for marketing.
How long we keep it
If your enquiry leads to an appointment, it becomes part of your record at the practice and is kept for as long as that record must be. Other enquiries are stripped of personal details after two years. What remains is an anonymous record of when and why people contacted us, which tells us nothing about you and helps us run the practice better.
Your medical records are separate and are held for the period the Health Professions Council requires.
How it is protected
Access is limited to the practice, accounts are protected by passwords and additional verification, and information travels to us encrypted. If information were ever exposed, we would tell you and the Information Regulator.
Measuring how the site is used
We use Google Analytics to see which pages people find useful. It records pages opened, the broad area a visit came from, the device and browser, and how you arrived. It does not tell us who you are, and we do not use it to identify you.
If you message us on WhatsApp
The WhatsApp button opens a conversation with the practice. That conversation is subject to WhatsApp’s own terms rather than this policy.
Your rights
You may ask what we hold about you, ask us to correct or delete it, or object to us using it. Phone 083 370 3312 or email reception@ckgeneralpractice.co.za.

Changes
If what we collect or where it goes changes, this page changes with it, and the date at the top tells you when.
